OCD: Developmental pathophysiology.

    Understanding disease mechanisms is key to earlier identification of disease and new treatment discovery. OCD displays aberrance in the same physiological processes and anatomical structures that are responsible for decision-making. In adolescents with OCD we found (1) impaired use of Pavlovian (predictive) cues to select instrumental actions, this was associated with task-related (fMRI) orbitofrontal hyperactivity which positively correlated with compulsion severity; and (2) perseveration despite devaluation of outcomes associated with ventromedial prefrontal cortex changes. Both sets of findings were associated with altered brain connectivity (diffusion MRI). Now we will investigate whether these changes are present in children under the age of 12 and their relatives.
